By submitting this form, I authorize SHALOM to create a personal and/or business profile for me in their secure, cloud-based system. I understand that communication will be conducted via the SHALOM client portal, and that the type of service I request will determine which system(s) I will have access to. All systems follow security protocols in accordance with applicable laws and regulations. I understand that some services are offered at a flat rate and must be ordered through SHALOM’s online service request link. This process ensures I receive all necessary instructions and requirements related to the requested service. For services not offered at a flat rate or not available through the online store, I may request an estimate before work begins. These services may be billed under SHALOM’s standard hourly rate of $125 per hour. I understand that SHALOM may require a deposit before beginning any billable work. Based on the nature of the services requested, I may also be asked to sign an engagement agreement and complete additional forms before services can begin.
